What are the different types of tummy tuck?

There is a whole range of different possibilities – we will be happy to advise you. Here are the individual components that can be influenced:

Skin

Different tightening patterns depending on where the excess skin is found. From lower abdominoplasty to tummy tuck with or without navel dislocation to extended tummy tuck and up to body lift. Different positioning of the scars depending on the preferred clothing – e.g.: thong. Individual variation of skin tension to achieve a beautiful waist.

Fatty tissue

Removal of fatty tissue with the skin – targeted removal of fat cells to improve lymphatic flow – tummy shaping – body shaping – liposuction of the flanks, the “love-handle”…

Fascia and muscles

The fasciae are the connective tissues that guide and hold the muscles. By shaping the fasciae like the abdomen is shaped from the inside. At the same time, the geometry of the muscular abdominal wall is restored – important after pregnancy or heavy weight loss, where previously the abdominal wall inside had also suffered. We analyse the existing changes and work out the better and more beautiful solution together with you.

Reasons for an abdominoplasty

Especially after heavy weight loss and pregnancies, many patients complain of sagging connective tissue, unsightly stretch marks and a hanging belly. In extreme cases, the excess skin can overlap and then chafe, the skin folds become moist with perspiration and eczema and inflammation develop.

An abdominoplasty can also eliminate large aprons of fat and folds of skin, as well as tighten the inner abdominal wall for which there are no other corrective options.

Lower abdominoplasty

In a lower abdominoplasty, the tightening is limited to the region below the navel. A much smaller incision is made and the navel remains in its original position. The result of the radiation is a considerably improved abdominal shape, and the waist is also emphasised again. In addition, the excess weight of the abdominal fat is removed.

Large abdominoplasty

In cases of severe overstretching of the inner abdominal wall, an extensive abdominoplasty with tightening also of the inner abdominal wall (fascia suture – muscle repositioning – fascia doubling) is often necessary. Hernias – abdominal wall hernias or an umbilical hernia – also have to be treated here time and again.

PD Dr. Max Geishauser is also a specialist surgeon and trained in the treatment of hernias. He is one of the pioneers of “component separation” in Germany and has developed special complementary techniques for this, which better preserve the strength of the abdominal wall.

Before the operation, we discuss the incision with you. The course of the scars depends on the amount and extent of fat to be removed. Individual wishes can also be taken into account.

If high leg cut-outs are worn for underwear or swimwear, the course of the scar can be steeper. Usually the incision is just above the pubic hair and runs in an arc from one pelvic bone to the other. As the belly button is corrected, but ultimately re-fitted at the same height as before, small, inconspicuous scars are also left around the navel.

Umbilicus reconstruction

A modern, new technique that can lead to excellent results is navel reconstruction. This allows aesthetic positioning and shaping of the umbilicus – and, if necessary, closure of umbilical hernias and correction of the rectus sheath.

Correction of rectus diastasis and the muscular abdominal wall

Due to the overstretching of the inner connective tissue and muscles after pregnancy or due to overweight, the geometry of the inner abdominal wall changes. As a result, the muscles do not find support during many movements and become weaker. Abdominal muscle training becomes much more difficult.

Here, the restoration of the functional structures can lead to a significant improvement in abdominal muscle function. And with it, trunk stabilisation and a reduction in spinal pain.

Operation

The abdominal tissue is detached up to the costal arch on the abdominal muscles, the excess is determined, the fatty tissue is removed and the straight also the lateral abdominal muscles are tightened. In this way the abdominal wall becomes firmer, can work properly again and the waist becomes narrower.

Anaesthesia – pain after abdominoplasty

An abdominoplasty takes 1.5 to 4 hours, depending on the findings and necessary corrections, and is usually performed under general anaesthesia. In addition, we block the pain nerves during and after the operation. This results in much less pain even after the operation. In most cases there is almost no pain at all, which considerably reduces the stress caused by the operation.

Aftercare

After the operation, you often feel a clear tension in the abdominal area. For the first 3 to 4 days, it is therefore helpful to give in to the tension and walk with a slight bend. However, as the slight swelling goes down and the tissues get used to the new tension, you can quickly walk straight again. Here we will help you correct your posture and show you an easy but effective abdominal workout.

As with almost all operations, good aftercare is important. Compression treatment of the trunk reduces swelling after the operation. It also leads to improved scar formation. Pressure treatment of fresh scars is one of the most effective methods for beautiful scar formation.

Although it may feel asif the compression girdle was providing good stability, the main effect is to allow skin and fat tissue to reconnect with the underlying muscles and fascia. Treatment with a well-fitted compression girdle should last up to 6 weeks. Some patients swear by continuing to wear it until the 12th week.

Aftercare of the scars and skin

Skin treatment with light creams is described as very pleasant by almost all patients. It restores moisture to the skin. The scars need more support as they are often rather dry in the beginning. Here we are happy to recommend good base creams. At first the scars will become more stable again and after a few weeks they will become softer and lighter. With the new suture techniques and suture materials, the overall scar quality has improved fundamentally.

Dressing change – stitches removed

With our special plastic surgery suturing techniques (“beauty suturing”), stitching is no longer necessary. We use special dressings with which you can shower immediately after the operation. A change of dressing is then only necessary after one week. This also leads to a much higher level of comfort after the operation.

The most important thing: walk – walk – walk

Exercise is especially important after a tummy tuck. This is because it makes the swelling disappear and gently massages the scars. Light exercise is good for the whole body, for example light walks on the flat. This gets the circulation going. In between, rest is good and helps the healing process.

Sport after a tummy tuck

As important as exercise is, stressing the abdominal wall should be delayed until the healing process has stabilised. Sports that put strain on the abdomen, such as cycling, should only be resumed after 6-8 weeks, depending on the operation and the time before the operation. Here, too, we will advise you individually.

Occupation – ability to work

The question of resuming professional activities depends on the stress of the work and the extent of the corrections. For example, desk work can usually be resumed after a few days. If the operation has a medical background, so that the health insurance company covers the medical part of the operation, a sick leave by the family doctor is possible, usually 4-6 weeks. Then any necessary physiotherapy or posture training can also be prescribed.

If the operation is only performed for aesthetic reasons (“cosmetic surgery”), sick leave is not possible. In this case it makes sense to take leave for the period of convalescence. We are happy to accommodate you with the choice of date for the operation so that official holidays can be used.
